Data Request Form

For reports/data that cannot be obtained using the CAT Tool, please complete the Information Request Form and submit as indicated.

For requests coming from outside the MOHLTC, the following guidelines apply:

  • Ten (10) working days are required to complete the request report
  • An Anonymous Information Agreement (AIA) between the Ontario Ministry of Health and Long-Term Care and the requester is required, should individual level Case Costing Data be requested. The AIA takes four (4) weeks to complete.  The following documents are required to go with the AIA agreement:
    • Liability Insurance Certificate which covers up to $2,000,000.00
  • Data requests that are for research purposes require the following documents:
    • The AIA agreement with Liability Insurance Certificate which covers up to $2,000,000.00
    • A Research Plan & Research Ethics Board (REB) Approval Letter
  • There is a service charge for each data request. The first hour is free of charge. Following the first hour, it is $100.00 per hour plus GST
